By sending this form, the student understands that:

The student will be charged the Holding Tuition during the break from lessons ($29 every 4 weeks)
Regular payments will resume automatically on the date of the 1st lesson after the student's break.
The completion of this form does not entitle the student to any refund of payments already processed or payments that are due in the 29 days after the date the form is received.
This is not a withdrawal form. If the student wishes to stop permanently their enrolment while On Hold, the student will need to fill a Withdrawal Form and comply with the provisions therein.
